All students will need to complete the following requirements. If the student degree program is completely online, please reach out to the Willcoxon Health Center for information.
Campus requirements include:
1. Complete medical health history form on Medicat.
2. An immunization record showing 2 doses of MMR vaccine (measles, mumps and rubella) on or after the 1st birthday and at least 28 days apart or a MMR titer (blood test) showing immunity for MMR (uploaded to Medicat). If born prior to 1957, you will not need to upload any documents, but you will still need to log into Medicat to complete other requirements.
3. Tuberculosis risk assessment questionnaire (completed on Medicat). *TB testing, if indicated, can be done at the health center with a charge to the student or outside TB tests can be uploaded to Medicat. If you have tested positive for TB in the past or been treated for TB, please reach out to the Willcoxon Health Center for documentation guidelines.
4. If living in dorm housing – proof of a meningitis (MCV4, Menveo, MenQuadfi or Menactra) vaccination on or after the 16th birthday
*If you have previously uploaded documentation to Med+Proctor, you still need to complete your account set up and medical health history form with Medicat. However, you do not need to upload any further records.
Once you have registered for classes, you should be able to set up your Health Center account.
We use a platform called Medicat to collect your health history and to communicate with you throughout your studies with our university.
Please set up your student portal with your MSSU email credentials.
You will need to have previously set up your Microsoft authenticator.
